Cookie Policy

What cookies and similar technologies this website uses, why we use them, and how you can control them. We do not use advertising or tracking cookies.

Effective date: October 21, 2026

1.What cookies are

Cookies are small text files a website stores in your browser. Similar technologies include browser storage (such as local storage) and the offline cache used when you install our website as an app. In this policy, "cookies" covers all of these.

2.Our approach

Eastern Virginia Advanced Care, LLC keeps cookie use to a minimum. We do not use advertising cookies, cross-site tracking, or analytics that build profiles of visitors, and we do not sell information collected through cookies. Non-essential cookies are only used after you allow them, and you can change your choice at any time.

3.Essential cookies (always on)

These are needed for the website to work and to keep it secure. They do not require consent under the GDPR or the ePrivacy rules because the site cannot function properly without them.

  • evac_cookie_consent: remembers your cookie choices. Set by EVAC. Lasts 180 days.
  • authjs.session-token: keeps authorized EVAC staff signed in to the administrative area. Only set when a staff member signs in. Lasts up to 30 days or until sign-out.
  • authjs.csrf-token and authjs.callback-url: protect sign-in forms from forgery and return staff to the right page after sign-in. Session cookies.
  • Hosting and security cookies: our hosting provider may set technical cookies to route traffic and protect the site from abuse. Session or short-lived.
  • Theme preference (local storage, "theme"): remembers the display setting. Stays until you clear browser storage.
  • App offline cache (Cache Storage, "evac-v1-static"): stores images and the offline page so the site loads quickly and shows a helpful page when you are offline. No form entries or personal information are cached.

4.Third-party content (only with your permission)

Our Locations page can show interactive maps from Google Maps. Google may set its own cookies (for example NID or AEC) and receive your IP address when a map loads. Maps stay hidden until you choose "Accept all", turn on "Third-party content" in Cookie Settings, or click "Load map". Google's use of this data is described in the Google Privacy Policy.

5.Online forms provided by JotForm

Our Client Intake and Referral pages use forms hosted by JotForm. When you open or submit one of these forms, JotForm may set cookies that are necessary to load the form, prevent spam, and submit your answers. These forms are part of the service you are requesting, and JotForm's handling is described in its own privacy policy.

6.Global Privacy Control

If your browser sends a Global Privacy Control (GPC) signal, we treat it as a choice to refuse non-essential cookies and do not show the cookie banner.

7.How to manage cookies

  • Use the "Cookie Settings" link at the bottom of every page to change or withdraw your consent at any time. Withdrawing consent is as easy as giving it.
  • You can also block or delete cookies in your browser settings. Blocking essential cookies may stop parts of the site, such as staff sign-in, from working.
  • To remove the installed app and its offline cache, uninstall the app or clear site data in your browser.

8.Changes to this policy

If we add new cookies or change how we use them, we will update this policy and, where consent is required, ask for your choice again.
